Team Health is looking for a nurse practitioner (NP) to work Monday through Friday doing primary care rounding in three skilled nursing facilities in the Lancaster, Pennsylvania, area.

Key Responsibilities:

* Perform primary care in a post-acute setting for short term and long-term patients

* Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care

* Order and interpret lab results

* Prescribe and monitor medications

* Consult with physicians and coordinate patient care with other healthcare practitioners

* Educate patients and their caregivers on coping skills to handle chronic and age-related conditions

* Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery

* Promote healthy lifestyle changes

Requirements:

* A minimum of 1 year experience as a nurse practitioner in a primary care setting required.

* Post-acute care experience a plus, but not required

* Active and unrestricted Pennsylvania NP license required

* Active and unrestricted DEA with prescriptive authority required

Estimated compensation range of $117,100 to $138,520 annually with no cap on productivity income potential.
