Ennoble Care is hiring a full-time Hospice RN Case Manager to visit patients in the Lancaster, York, and Lebanon County, PA region. As a member of the interdisciplinary team, the RN Case Manager conducts skilled nursing assessments, plans care, and coordinates services to maximize comfort and health for patients and their loved ones.
You will document in the EMR, collaborate with physicians and hospice staff, educate families about levels of care, and participate in care conferences.
