Hospice Registered Nurse

Canonsburg, PA (supporting patients in Canonsburg and surrounding areas)

This ongoing role requires 40 hours per week, 8‑hour shifts, as a staff RN in our home health and hospice program. Registered Nurses collaborate to manage and deliver patient care plans, providing excellence in a team‑based environment.

• Provide and coordinate hospice care for patients within the assigned geographic area.
• Manage patient care plans and maintain accurate charting and documentation.
• Use tablets, smartphones, and EMR software to record care data.
• Drive up to 20% of the day to patient locations.
• Meet critical documentation deadlines and maintain high quality of care.

• RN license in the state of practice.
• Associate degree or higher from an accredited School of Nursing.
• Two years of RN experience; hospice experience preferred.
• Current driver’s license and ability to spend approximately 20% of the day driving to patient locations.
• Commitment to consistently meet critical deadlines for charting.
• Ability to self‑manage time and schedule.
• Demonstrated experience with tablets, mobile phones, and EMR software.
