County Caseworker ; LG Supports Coordinator - Wayne County MH

Located in Honesdale, Pennsylvania, Wayne County wants you to make a difference every day! We are seeking a compassionate, community‑minded professional to join our team as a Supports Coordinator.

• Service Planning: Develop Individual Support Plans and review them regularly to match each person’s needs
• Support Coordination: Connect individuals with natural supports and necessary services based on their goals
• Community Engagement: Encourage participation in community activities and employment when appropriate
• Field Visits: Travel to meet individuals and teams in person to review progress and needs
• Record Management: Complete documentation and data entry to maintain accurate and timely records

• Full‑time employment
• Work hours 8:00 AM – 4:30 PM, Monday‑Friday, with a 1‑hour lunch
• Occasional overnight and out‑of‑county travel for meetings or training
• Coordinators are not on call, but may be called out in an emergency
• Telework: No telework option for this position

• Six months of experience as a County Caseworker 1 or successful completion of the County Social Casework Intern program
• A bachelor’s degree with a social welfare major
• A bachelor’s degree supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency
• Any equivalent combination of experience and training that includes 12 college credits in the fields listed above
• Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree or credits.

• PA residency requirement is currently waived for this title
• You must be able to perform essential job functions

• A conditional offer of employment will require a drug screening
• You must pass a background investigation
• This position falls under the provisions of the Child Protective Services Law
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance
