Crawford County Health & Human Services is seeking a compassionate, dedicated individual to join our team as a Birth to 3 and Children’s Long‑Term Support (CLTS) Social Worker. This position plays a vital role in supporting children with developmental delays or disabilities and their families. The ideal candidate will bring strong interpersonal skills, a deep sense of empathy, and a commitment to partnering with families to promote each child’s growth, independence, and well‑being.
In this role, you will coordinate services within Wisconsin’s Birth to 3 and CLTS Programs, conduct developmental assessments, and help families access individualized supports to meet their child’s needs.
Key Responsibilities- Conduct home visits to assess developmental strengths, delays, and family needs for children ages Birth to 3.
- Complete intake and eligibility processes for the Birth to 3 Program and the CLTS Waiver.
- Develop individualized service plans that support the child’s developmental goals and the family’s priorities.
- Coordinate CLTS services such as therapies, adaptive equipment, environmental modifications, and other supports.
- Collaborate with early intervention providers, medical professionals, childcare programs, schools, and community partners.
- Support families in understanding developmental milestones, available services, and long‑term support planning.
- Maintain accurate documentation, service authorizations, progress notes, and reports in accordance with state and county standards.
- Advocate for the needs of children and families, helping connect them with resources that promote stability and resilience.
- Participate in intake and on‑call rotations as required
