Position: Caseworker 2

POSITION SUMMARY

An individual in the Caseworker position is assigned to assist participants that are prospective or currently enrolled in one of our case management units below. Each case management unit serves a more focused population and coordinates appropriate level of services based on the participant’s needs and success in everyday life.

Early Intervention - Assist families and children aged 0-3 with developmental delays and/or disabilities to access, understand, and receive appropriate services throughout the early intervention process from intake to transition out of the program.

Intellectual Disabilities/Autism - Locate, coordinate and monitor services for those enrolled in the /A program aged 3 to end of life.

Administrative Entity - Assists in the operations of the delegated functions dictated by the Office of Developmental Programs for eligible waiver and prospective waiver participants in PA.

Mental Health Blended - Assists no more than thirty (30) adults (18+) with serious and persistent mental illness or children and adolescents with or at risk of a serious emotional disturbance.

Forensic Mental Health Blended - Assists no more than thirty (30) adults with serious and persistent mental illness or children and adolescents with or at risk of a serious emotional disturbance that may be enrolled in a wellness court, incarcerated, and/or reintegrating back into the community after incarceration.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Provide case management services.
  • Responsible to maintain face-to-face and telephone contact with assigned consumers and collaterals at the appropriate time frames as outline by federal, state, and local regulation.
  • Responsible to spend 75% of working hours in contact with assigned persons and collaterals, including travel time.
  • Responsible to spend at least 50% of working hours outside of the office.
  • Responsible to spend no more than 25% of working hours attending meetings and completing paperwork.
  • Responsible to participate in individual and group supervision as scheduled.
  • Responsible to attend training opportunities designated by OMH for Intensive Case Managers and other training as directed by the County Programs.
  • Participate in officer coverage duty as required during regular working hours.
  • Perform related work as required.
ESSENTIAL REQUIREMENTS
  • A bachelor's degree with a social welfare major; or a bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences with a combination of experience and training which includes one year of case management experience in a public or private social services agency; or six months of experience as a County Caseworker 1;

    or any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
  • Knowledge of current social case management principles, techniques, and methods.
  • Knowledge of current social, economic, and health problems and resources and the recognition of their impact on the growth and development of people.
  • Knowledge of individual and group development and behavior and ways of working effectively with adults and children who have social, economic, emotional, or health problems.
  • Knowledge of the basic principles and methods of program interpretation and community organization.
  • Knowledge of social welfare policy and law as they relate to agency function and purpose and societal structures.
  • Ability to work effectively with people and aid them to grow in the constructive use of their potential in adjusting to their specific problems.
  • Ability to understand and accept the needs and rights of others and to work with adults and children who are physically challenged, emotionally troubled, or economically disadvantaged.
  • Ability to conduct individual and family interviews and to use them to identify individual and family problems.
  • Ability to establish and maintain effective working relationships with clients, their families, other staff, outside agencies and institutions, and the general public.
  • Ability to plan and organize work, prepare adequate records and reports, set priorities, and maintain a caseload in an effective and timely manner.
  • Ability to plan, organize, and direct the work of others.
  • Ability to interpret and apply relevant laws, regulations, and policies governing agency services.
  • Ability to adequately express ideas orally and in writing.
