• Perform visits and complete coordination of client care.
• Act as point of contact for all disciplines involved with providing care to patients and oversee the frequency of visits for the episode.
• Consult as needed with the physician and the office, providing details about patient care.
• Perform accurate OASIS collection, ensuring the medication profile remains current and lab values reported to the physician timely.
• Attend weekly case conferences and monthly case manager meetings.
• Hybrid position offering the opportunity to work in both home health and hospice settings, supporting patients and families throughout the full care journey.

• Graduate of an approved school of clinical education.
• Licensed in the state where currently practice.
• At least one year experience as a licensed professional.
• Basic demonstrated technology skills, including operation of a mobile device.

• Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director.
• Medicare home health or hospice experience is preferred.

• Possess a valid state driver license.
• Maintain automobile liability insurance as required by law.
• Maintain dependable transportation in good working condition.
• Be able to safely drive an automobile in all types of weather conditions.
• Possess CPR certification for the healthcare provider.
