Per Diem RN – Overview

The Per Diem RN is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit.

Qualifications
  • Current RN license in the state where the employee will be working.
  • Active BLS certification for healthcare professionals from the American Heart Association or Red Cross.
  • Minimum one (1) year of nursing experience, hospice or hospital experience preferred.
  • Previous experience working with an EMR/EHR system.
  • Mobile Driver – valid driver’s license and automobile insurance per company policy.
  • Reliable transportation to meet visit schedule.
  • Ability to use equipment with visual and auditory mechanisms.
  • Effective communication in English (verbal and written).
  • Flexible schedule to meet the needs of Hospice House patients.
  • Ability to manage emotional stress of working with and caring for terminally ill patients and their families.
  • Physical ability to access patient rooms, other areas of the hospice house/hospital/ALF, and any home (navigate stairs, narrow spaces, tolerate heat and lack of air conditioning) within any part of the service area.
  • Ability to perform essential functions and physical requirements (lifting patients/equipment, bending, pushing/pulling, kneeling) with or without reasonable accommodation.
  • For employees working at PACE – completion of Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs.
  • In some locations – cross‑training to hospice house or field positions of equal licensure/certification and scope when census calls for flexible staffing.
Responsibilities
  • Provides and manages direct care to patients and families as part of the Interdisciplinary Group, incorporating psychosocial, spiritual, cultural, physical, and biological components, and appropriate nursing interventions and follow‑up.
  • Coordinates the Plan of Care, ensuring an individualized plan accurately reflects the patient’s evolving needs.
  • Educates patient, family, caregivers, and other health professionals about disease process, decline, palliative interventions, caregiving, dying process, and safety practices.
  • Reports changes in the patient’s condition to appropriate IDG or other health professionals.
  • Participates with IDG to evaluate hospice referrals/admissions for level of care appropriateness.
  • Attends patient deaths; prepares body for transport; oversees disposal of medications.
  • Provides appropriate bereavement interventions to the family and caregiver in conjunction with psychosocial staff.
  • Prepares concise oral and written reports to IDG; encourages input from all disciplines.
  • Communicates accurately and completely to physicians, staff members, patients, families, and supervisors; utilizes positive approaches when working with others.
  • Supervises patient care given by other nursing team members (LPNs, hospice aides, contract nurses).
  • During emergencies (e.g., hurricanes) reports to work at designated locations to ensure continuity of services, including possible advance reporting and overnight stays.
  • Performs other duties as assigned.
  • Completes clinical admissions for all Company services, including explaining services to potential patients/families, addressing concerns, educating on admission requirements, completing informed consents and initial assessment, initiating Plan of Care, and initiating skilled nursing interventions to prevent complications.
  • Completes documentation per Company policy, acts as Company representative at assigned facilities, facilitates referrals, communicates with Admissions for timely follow‑up, informs Admissions when assignment is complete, prepares for next assignment, provides all necessary clinical communication using SBAR, discusses potential needs with after‑hours staff, obtains consents for services, arranges safe discharge…
