Role
The Admission RN assesses patients in their place of residence or in hospitals and initiates care into the services of Chapters Health System, ensuring a seamless transition to Care Navigation and serving as an ambassador of the organization.
Qualifications- Current RN license in the state of employment.
- Minimum 1 year nursing experience (hospital or hospice preferred).
- Experience with an EMR/EHR system.
- Strong clinical assessment skills, preferably with emergency department or ICU background.
- Ability to communicate effectively in English; bilingual in Spanish preferred.
- Professional presentation skills.
- Valid driver’s license and automobile insurance per company policy (Mobile Driver requirement).
- Strong goal/outcome orientation and time‑management skills.
- Active BLS certification for healthcare professionals.
- Completes clinical admissions for all company services.
- Explains services to patients/families, addresses questions, provides reassurance.
- Identifies admission objections and barriers, provides responses.
- Educates patient/family on admission requirements and addresses immediate needs.
- Completes informed consents and initial assessment per regulations and company policy.
- Provides information to physicians and initiates Plans of Care for immediate needs.
- Initiates skilled nursing interventions to prevent complications, alleviate symptoms and maximize comfort.
- Obtains physician orders and completes documentation per company policy.
- Acts as Company representative at assigned facilities, facilitates referrals, works closely with referring staff.
- Communicates frequently with Care Navigation staff to ensure timely follow‑up.
- When assignment is complete, informs Care Navigation team and prepares for the next assignment.
- Provides all necessary clinical communication timely using SBAR.
- Obtains consents for services and arranges safe discharge efficiently while meeting productivity expectations.
- Obtains medical record information for the referral.
- Develops strong relationships with case managers, physicians, and other providers.
- Supports education on all available services with Professional Relations Representatives.
- Provides care for any GIP patients in hospital, including daily visits, professional management, documentation, and efficient discharge.
- During emergencies (e.g., hurricanes), may be required to report to an alternate location, stay overnight, and maintain service continuity.
- Performs other duties as assigned.
Requires the ability to stand, sit, walk, use hands, lift and carry, bend, kneel or climb stairs as necessary. Must be able to perform other tasks as required by the job.
CompensationPay range: $76,612.89 – $.
