LMSW Case Manager, Social Work

Kaiser Permanente is seeking an LMSW Case Manager to support inpatient members in Lanham, MD. In this role, you will conduct psychosocial assessments, develop care and discharge plans, and coordinate services across medical, behavioral health, and community resources.
