LMSW Case Manager, Social Work

Kaiser Permanente is seeking an LMSW Case Manager to support inpatient members in Lanham, MD. In this role, you will conduct psychosocial assessments, develop care and discharge plans, and coordinate services across medical, behavioral health, and community resources. You will collaborate closely with physicians, nurses, and allied health professionals to ensure safe transitions of care, reduce readmissions, and support patient and family understanding of treatment.

The position requires strong documentation skills, EHR proficiency, and a commitment to continuous improvement and patient-centered care.

Responsibilities

• Conduct comprehensive psychosocial and risk assessments for inpatient members
• Develop and implement individualized care and discharge plans
• Coordinate services with physicians, nurses, and allied health professionals
• Arrange post-acute services, community resources, and follow-up care
• Provide crisis intervention and short-term counseling as needed
• Advocate for patient needs and support family understanding of care plans
• Ensure accurate and timely documentation within EHR systems
• Participate in interdisciplinary rounds and care conferences
• Monitor outcomes to reduce readmissions and improve care transitions
• Contribute to quality improvement initiatives and best practice development
