Foundations for Home and Community seeks a Therapeutic Foster Care Social Worker to support children and youth in foster homes across Minnesota. This role provides intensive, trauma-informed case management, develops individualized service and safety plans, and coordinates therapeutic supports with foster parents, schools, county workers, and clinicians. The Social Worker will conduct home visits, crisis intervention, and ongoing assessments while maintaining timely, accurate documentation.
You'll collaborate closely with a mission-driven team, receive supervision toward licensure, and help youth achieve stability, permanency, and long-term wellbeing.
Responsibilities
- Provide intensive, trauma-informed case management for children in therapeutic foster homes.
- Conduct regular home visits to assess safety, stability, and progress toward goals.
- Develop and monitor individualized service, treatment, and safety plans.
- Coordinate services with foster parents, county workers, schools, and mental health providers.
- Respond to crises, provide short-term stabilization, and connect families to needed resources.
- Maintain accurate, timely documentation in accordance with agency and county requirements.
- Participate in team meetings, case consultations, and care coordination with community partners.
- Educate and support foster parents in behavior management, trauma-informed approaches, and advocacy.
- Ensure compliance with child welfare regulations, licensing standards, and agency policies.
- Engage in ongoing professional development and reflective supervision.
