Health and Wellness - Independent Living Nurse (RN or LPN)PRN, Weekdays and Weekends
Position SummaryThe Independent Living Nurse provides clinical oversight, health monitoring, and wellness support for residents living independently within the community. The nurse promotes resident autonomy while ensuring timely identification of health changes, coordination of care, and education that supports safe, healthy aging in place.
SchedulePRN position with availability needed on weekdays and weekends between 8AM and 7PM.
Key Responsibilities- Conduct wellness checks and health assessments for Independent Living residents as needed.
- Assist residents with navigating healthcare needs, including appointments and follow‑up care.
- Provide medication education and support safe self‑administration practices.
- Support transitions of care following hospitalizations or rehab stays.
- Provide health education on chronic conditions, fall prevention, nutrition, and wellness topics.
- Participate in community wellness programs, screenings, and resident education events.
- Respond to resident emergencies and provide first aid or triage as needed.
- Identify safety risks in resident environments and recommend interventions.
- Maintain compliance with state regulations and organizational policies.
- Active RN or LPN license in the state of PA.
- Current CPR certification.
- Experience in senior living, home health, community health, or related setting.
- Experience with chronic disease management or wellness programming.
- Strong assessment, communication, and documentation skills.
- Ability to work independently and manage a flexible schedule.
