Job Overview
With a commitment to patient‑ and family‑centered care, the professional nurse uses the nursing process in ways that value the uniqueness of each individual and address the physical, psychological, emotional, and social needs of the diverse patients served by Jefferson Health. The nurse develops nursing plans of care with the patient and their designated care partner(s) to effectively address individualized needs and priorities, in alignment with their cultural beliefs and values.
Responsibilities- Interact with co‑workers, patients, their family or designated care partner(s), and other staff in a manner consistent with Jefferson values.
- Integrate evidence‑based practice, clinical expertise, patients’ and their family or designated care partner(s)’ preferences and cultural beliefs and values for the delivery of optimal healthcare.
- Partner with patients and their family or designated care partner(s) in identifying learning needs and priorities; provide education in ways that are useful, understandable, and affirming.
- Minimize risk of harm to patients and others by adhering to established nursing practices and protocols and individual performance standards.
- Use information technology to communicate, manage knowledge, mitigate error, and support ethical decision making.
- Use data to monitor care process outcomes and apply improvement methods to design and test changes that continuously enhance quality and safety of patient care.
- Function effectively within nursing and inter‑professional teams, fostering open communication, mutual respect, and shared decision‑making to achieve quality patient care.
- Recognize patients and their family or designated care partner(s) in providing safe, compassionate, and coordinated care based on respect for their personal preferences, priorities, needs, and cultural beliefs and values.
- Create an environment of belonging and inclusion, demonstrating problem‑solving and conflict‑resolution skills.
- Demonstrate commitment to learning and seek opportunities for continued professional growth and development; act in a primary preceptor role for appropriate staff members.
- Actively participate in hospital‑ and practice setting‑based committees, including safety and quality improvement.
- Demonstrate willingness to give and receive feedback and integrate appropriate changes into practice.
- Specialized diploma graduate of an accredited school of nursing required.
- Proficiency in patient‑and family‑centered practice; ability to convey respect and collaborate with each patient, family, and/or designated care partner in care planning.
- Cultural awareness; ability to convey respect and connect effectively with people from diverse backgrounds.
- Effective communication skills; understanding of communication concepts, tools, and techniques, and ability to transmit, receive, and interpret information correctly.
- Ability to delegate to other team members appropriately and respectfully.
- Knowledge of patient assessment and ability to implement a treatment plan.
- Knowledge of child and adult education techniques; ability to elicit learning priorities and educate patients and their designated care partners on conditions, treatments, medications, and recovery steps.
- Knowledge of medication administration and prescription, including use of various medications for patient treatment.
- Knowledge of discharge planning, including post‑discharge needs and follow‑up care coordination.
- Knowledge of nursing assessment, diagnosis, clinical judgement, and generating resolutions based on available resources.
- Knowledge of health information documentation and appropriate methods to record medical data in written or electronic form.
- Basic Life Support (BLS) certification.
- Certification from the American Heart Association and RN licensing in Pennsylvania or New Jersey.
- Bachelor’s degree in nursing or completion of BSN within 3 years of hire strongly preferred.
- One year of previous RN experience in an acute care setting strongly preferred.
- Active RN license required in the state of practice.
