Job Overview

Advanced Practice Clinician (Nurse Practitioner/Physician Assistant) within the acute care hospital setting works in collaboration with a multidisciplinary health care team and provides comprehensive care on the inpatient Hospital Medicine service.

• Assessment & Management: Performs complete history & physicals, documents in the medical record. Orders & collects data using appropriate assessment techniques, supporting diagnostic information and procedures. Serves as first responder for patient status changes, orders, and procedural/resuscitative interventions. Assesses for risks associated with acute and complex chronically ill patients (medication side effects, immobility, impaired nutrition, fluid & electrolyte imbalance, immunocompetence, invasive interventions, diagnostics).
• Diagnosis: Collaborates with interdisciplinary team to make diagnoses of acute and complex chronic conditions. Manages diagnostic tests through ordering, interpretation, performance, and supervision. Formulates differential diagnoses by priority. Diagnoses complications and orders appropriate interventions.
• Plan of Care: Identifies expected outcomes, formulates & documents a plan of care to address complex acute & chronic needs. Utilizes evidence‑based guidelines and protocols, implements and modifies the plan, prescribes diagnostic strategies and therapeutic interventions, and may perform advanced procedures consistent with privileges and competency validation.

  Maintains ongoing communication with interdisciplinary team. Facilitates and communicates with patient, family, and staff to promote continuity across inpatient and outpatient continuum.
• Documentation: Documents/dictates key components via daily progress notes, transfer, discharge summary, and/or clinic notes (H&P, daily progress, plan of care, problem lists, procedure notes, acute event, discharge, death summaries) in the medical record per unit/service. Documentation is timely, meets compliance standards, and captures patient acuity.
• Professional Practice: Demonstrates professional practice behaviors (preceptor/mentoring, education of graduate students). Seeks research opportunities, participates in advanced practice grand rounds & specialty meetings, maintains CEUs and membership in professional organizations.

• Master’s Degree
• National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant
• BLS certification
