Independent Living Nurse - RN Join the team at Living Branches! Come Live & Grow – Together!

As the Independent Living Nurse (RN) at Living Branches you are coordinating various tasks involved in providing services for the Residential Living residents to promote the health and well-being of each resident. This includes providing accurate nursing assessments, facilitating services and operating a wellness clinic.

Independent Living Nurse (RN):
  • Completes updated assessments for all Residential Living residents on an annual basis or following a significant change in residents' health status
  • Works with residents and their families throughout the decision-making process concerning the need for transitioning within the continuing care community
  • Obtains physician orders as necessary for residents. This includes coordination of medication orders, lab work, therapy services, consultations and other physician ordered services as needed
  • Conducts open walk-in clinic in the Wellness Center Monday through Friday
  • Maintains records of residents seen (in clinic and in residence) and services rendered and documents to electronic file, Point Click Care (PCC).
  • Coordinates the residents use of in-home support services as needs are identified. This includes internal and external providers of service
  • Works with Director of Residential Living Services to expand growth of wellness education programs and service options for Residential Living residents and supports the promotion of the wellness philosophy
  • Registered Nurse license (required)
  • 2-4 years of nursing experience (required)
  • Experience providing care for geriatric persons or working with dementia residents preferred
  • Knowledge of HIPAA rules and regulations and of regulations and guidelines governing long-term care
  • Ability to encourage and build mutual trust, respect, and cooperation among team members
