Maternal Program Specialist and Lansdale Site Coordinator (On-site, full-time)

  • Location: ACLAMO Norristown and Lansdale
  • Schedule: Monday through Friday, some evenings and Saturdays as required
  • Reports to: Health and Wellness Program Manager
Job Description

The Maternal Program Specialist & Site Coordinator is responsible for overseeing ACLAMO’s maternal program while coordinating service delivery at the Lansdale site in alignment with applicable grant and contract requirements. This role provides comprehensive case management, benefits navigation, education, community engagement, and maternal-focused programming, while ensuring the Lansdale site effectively serves all community members, with particular attention to pregnant women, new mothers, and caregivers when applicable.

This position supports ACLAMO’s mission by improving access to maternal services, benefits, education, and coordinated care, and by fostering equity and community well-being in Montgomery County.

Role and Responsibilities Maternal Program Implementation
  • Conduct proactive outreach to recruit and engage pregnant women and new mothers.
  • Provide comprehensive case management, including assessments, care planning, referrals, follow-up, and coordination of services through home and office visits.
  • Educate families on SNAP, health insurance, and other benefits as part of maternal case management.
  • Provide information and referrals related to prenatal care, parenting resources, healthcare, and community services.
  • Lead special maternal projects and activities, including support groups, caregiver workshops, car seat and diaper initiatives, and maternal-focused community events.
Education & Community Engagement
  • Organize and facilitate women's support circles and caregiver workshops aligned with Healthy Fridays.
  • Provide group education sessions and life skills training related to maternal and child well-being.
  • Collaborate with healthcare providers, social service agencies, and community partners to strengthen the maternal support network.
  • Support maternal-focused outreach efforts and community engagement activities.
Lansdale Site Coordination (Grant-Funded Programs)
  • Oversee day-to-day coordination of services at the Lansdale site.
  • Serve as a first point of contact for all clients accessing services at the Lansdale location, regardless of program.
  • Provide direct assistance when appropriate and triage clients to the appropriate ACLAMO staff or external partners when needs fall outside the scope of the maternal program.
  • Provide targeted support and referrals for maternal clients when applicable.
  • Maintain and develop relationships with community members and partner organizations.
  • Ensure alignment with applicable grant and contract goals, deliverables, and reporting requirements.
Documentation, Data, & Reporting
  • Maintain accurate and timely case notes, client files, outreach records, referrals, and program participation documentation.
  • Enter data, submit applications, and complete reports in a timely manner to support grant compliance and monthly reporting.
  • Ensure confidentiality, data integrity, and compliance with ACLAMO policies and applicable regulations.
Organizational Responsibilities
  • Attend ACLAMO staff meetings, trainings, and required professional development activities.
  • Support program-related outreach, education, and special initiatives as assigned.
  • Perform other duties as assigned.
Qualifications & Education Requirements
  • Bachelor's degree in human services, Social Work, Public Health, Community Health,Sociology, or a related field or an equivalent combination of education and relevant
    experience.
  • Minimum of two (2) years of experience in maternal and child programs, community services, benefits navigation, case management, or social services.
  • Must be fluent in Spanish and English.
  • Experience working with underserved and immigrant communities preferred.
  • Proficiency in Microsoft applications (Outlook, Word, Excel, PowerPoint).
  • Experience with data management systems, including accurate data entry, tracking, and reporting.
  • Demonstrated experience in case management and community outreach, particularly in maternal and child programs.
