Merakey is seeking a community-based Blended Case Manager in Colmar, PA to support adults and children in Montgomery County. You will assess needs, develop service plans, coordinate with providers, and help clients access community resources to improve stability.
The role requires traveling to homes and community sites, managing a caseload of 25–30 clients, and documenting progress while adhering to agency policies. Monday–Friday, 9am–5pm, with optional on-call shifts.
