About the Role:

As an Occupational Therapist with Twin Oaks Health & Rehab, you will be responsible for designing and implementing individualized treatment plans to help our residents and patients achieve their maximum functional potential. This may include conducting initial assessments, creating and updating treatment plans, and providing hands-on therapy sessions. You will work closely with our interdisciplinary team to ensure that our residents and patients receive the highest level of care and support.

Responsibilities:
  • Design and implement individualized treatment plans to meet the unique needs of our residents and patients
  • Conduct initial assessments and re-evaluations to monitor progress and adapt treatment plans as needed
  • Provide hands-on therapy sessions to improve functional abilities, mobility, and overall well-being
  • Collaborate with our interdisciplinary team to ensure comprehensive care and support
  • Maintain accurate and detailed records of treatments and patient progress
  • Participate in staff development and education to stay current with best practices and industry developments
