Optimal Care - Evergreen in Michigan is seeking a Hospice Medical Social Worker to deliver compassionate psychosocial care to patients and families navigating end-of-life challenges. You will assess, counsel, and support advance care planning as part of the interdisciplinary team. This role requires an MSW and state license, with travel to patients' homes, facilities within Lansing and Jackson areas.
Hours are Monday to Friday, 8 a.m. to 5 p.m.
