Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to deliver in-home health evaluations for members of our health plans. The visits last about 45 minutes and include medication and medical history reviews, a physical evaluation, and optional tests per the health plan.
You will travel locally and within licensed states, and may conduct virtual visits as needed. The role is part-time with an hourly base rate plus incentive structures.
