• Complete admissions, social services evaluations, provide referrals, and maintain communication with residents and families.
• Assist guests with financial and legal matters, such as applying for pensions, referrals to lawyers, and funeral home arrangements for pre‑planning.
• Provide discharge planning services, including placement on waiting lists for community congregate living, arranging intake for home care, transfer arrangements to other facilities, and coordination with outside agencies for safe discharges.
• Provide or arrange counseling services and psychiatric medication management as indicated.
• Ensure documentation is accurate and timely in MDS care plans.

• Current social work licensure in the state.
• Bachelor’s degree in social work required; master’s degree preferred.
• One year of supervised social work experience in a health care setting preferred.
