About WellvanceWellvance (formerly AuSable Valley CMH) provides state-funded mental health services to residents of Iosco, Ogemaw, and Oscoda counties, serving individuals with Intellectual/Developmental Disabilities (I/DD), Serious Persistent Mental Illness (SPMI), and Serious Emotional Disturbance (SED).
Position OverviewThe Case Manager supports individuals with SMI, SED, and/or I/DD by coordinating services that promote independence, community inclusion, and overall well-being. This role involves advocacy, service linkage, and monitoring to ensure continuity of care and prevent service gaps.
Key Responsibilities- Develop and implement person-centered service plans
- Link individuals to medical, social, educational, and therapeutic resources
- Coordinate and monitor services to achieve successful outcomes
- Provide advocacy and support to reduce barriers to care
- Participate in emergency on-call rotation
- Bachelor’s degree in a human services field
- Minimum one year of experience in mental health
- Certification as QMHP, QIDP, and/or CMHP, or Michigan licensure (limited or full)
- Strong organizational, communication, and problem-solving skills
- Ability to work flexible hours and travel within the service area
- Valid Michigan driver’s license, vehicle insurance, and satisfactory driving record
