• Evaluate occupational therapy patients to determine rehabilitation needs and develop a plan of care.
• Participate in the implementation and development of the Plan of Care to ensure quality, continuity, and proper discharge planning.
• Conduct initial therapy evaluation visits and re‑evaluate patient therapy needs during each visit.
• Perform follow‑up assessments according to OASIS and PPS regulatory guidelines.
• Communicate significant findings, problems, and changes in condition or environment to the appropriate supervisor, physician, and other team members.
• Initiate plans for patient safety using patient, family, and community resources.
• Verify the Plan of Care prior to each visit and provide care according to physician orders, assessment data, and established standards and guidelines.
• Write physician orders to cover additional visits and changes to the Plan of Care, per agency policy.
• Incorporate patient care goals into therapy sessions as evidenced by documentation in therapy notes.
• Perform occupational therapy services in accordance with accepted standards of practice and certified by the patient’s physician.
• Instruct and provide patients and/or families with activities of daily living using adaptive equipment and adaptive sensory integration/fine motor coordination skills.
• Assist patients and/or caregivers in securing necessary equipment, including fabrication of custom‑made splints.
• Instruct patients and/or caregivers in the therapy home program, providing modification and clarification when necessary.
• Counsel and instruct patients and families in meeting therapy and related needs.
• Supervise and/or provide clinical direction to Certified Occupational Therapy Assistants (COTA), Home Health Aides (HHA), and clinical staff to ensure the therapy care plan is followed and quality care is provided.
• Prepare for a physically demanding, high‑stress environment: full range of body motion, handling, lifting, and transferring patients.
• Potentially work irregular hours, including call hours if applicable.

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one (1) year of experience in a clinical care setting.
• Beyond basic requirements, must be able and willing to travel within the branch/office coverage area.
• Must have a dependable vehicle, a valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel 50% of the time.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
