CURRENT HOURS AND STARTING TIME: Full-time (37.5 hours per week) position. The current start time for this position is 8:30 a.m.
General Summary Under direction, provides direct service to people served, family members, and designated others. Implements program activities, participates in research & program evaluation, and provides statistics as requested by administration. Link, monitor, and coordinate services for persons served. Coordinate care with physical health care provider(s). Familiarity of community resources. Develop plan of service using person-centered planning process.
Essential Functions And Responsibilities Conducts assessments, service planning, specialized and intensive case management, living skills, training, crisis intervention, advocacy, linking and monitoring for assigned persons served. Performs home and community-based interventions. Maintains department productivity and quality measures. Complies with Macomb County Community Mental Health (MCCMH) documentation requirements in a timely manner with use of electronic medical record. Completes required MCCMH trainings and meets continuing education requirements as determined by professional licensing board.
Coordinates with family, aftercare programs, and ancillary support services to secure aftercare. Participates in staff meetings, in-service training, and supervision with the designated Clinical Supervisor. Monitors services for individuals served to evaluate effectiveness through face-to-face contact, facility visits, consultation with family members, significant others, service providers, and records review. Measures the effectiveness of interventions to determine outcomes and identify needs of the individuals served.
Operates an automobile while performing assigned job duties. Performs related duties as assigned.
Qualifications
Required Education and Experience
- Bachelor’s degree from an accredited college or university in Psychology, Social Work, Special Education, Counseling, or other mental health related field
- Meet qualifications as specified in the Michigan Department of Health and Human Services (MDHHS) Prepaid Inpatient Health Plan/ Community Mental Health Services Program (PIHP/CMHSP) Qualifications per Medicaid Services as a Child Mental Health Professional (CMHP) below:
- Individual with specialized training and one (1) year of experience in the examination, evaluation, and treatment of minors and their families and who is a physician, psychologist, licensed or limited-licensed master’s social worker, licensed or limited-licensed professional counselor, or registered nurse; OR
- An individual with at least a bachelor’s degree in a mental health-related field from an accredited school who is trained and has three (3) years of supervised experience in the examination, evaluation, and treatment of minors and their families; OR
- An individual with at least a master’s degree in a mental health-related field from an accredited school who is trained and has one (1) year of experience in the examination, evaluation and treatment of minors and their families. For the Behavioral Health Treatment/Applied Behavior Analysis (BHT/ABA) services individuals must be a Board Certified Assistant Behavior Analyst (BCaBA), Board Certified Behavior Analyst (BCBA), or Psychologist working within their scope of practice with extensive knowledge and training on behavior analysis and BCBA certified by 9/30/2020.
