The Assertive Community Treatment Team Leader/Supervisor provides clinical leadership to the ACT team, understands and communicates ACT eligibility and program requirements, adheres to MDHHS, Medicaid, CARF and NCCMH standards, and provides direct services to clients. The ACT Supervisor strives for positive outcomes for clients that include reduced hospital utilization, improved psychosocial functioning, and achievement of person-centered goals. This position coordinates the ACT program with the MI service system, other agency programs and in the community.
RESPONSIBILITIES:- Supervises the ACT program staff, activities, assignments, productivity, utilization of team resources, and quality and cost-effectiveness of services.
- Participates in team discussions, planning, and establishment of goals for person-centered plans including the provision of integrated treatment planning for consumers with co-occurring mental health and substance use disorders.
- Coordinates and participates in on call schedule and consultation for team members during "after hours" programming.
- Coordinates services with other MI Programs, i.e., outpatient, Clubhouse, North Country Residential and adult service coordination.
- Coordinates psychiatric, medical services and consultation for ACT clients.
- Maintains positive relationships with community providers.
- Maintains confidential program and client reports, records and data.
- Provides day-to-day program management including office and supply management and vehicle maintenance.
- Manages emergency assistance funds and petty cash.
- Provides outreach and skill training to identified client population.
- Coordinates ACT referrals and planning with intra-agency and hospital liaison staff.
- Visits clients at their residence or within the community.
- Oversees staff clinical documentation requirements.
- Reports service activity in accordance with agency policy, procedure and Medicaid requirements.
- Ensures the ACT program meets safety, MDHHS, Medicaid, CARF, CMH and other regulatory requirements.
- Utilizes approved intervention techniques based on Crisis Prevention Institute training.
- Master’s degree in Social Work, LPC or a licensed human services discipline; with appropriate licensure or certification to provide clinical supervision.
- Must qualify as a mental health professional in accordance with MDHHS and professional standards. Two or more years of experience with persons with serious mental illness. At least two years of supervisory experience.
- Satisfactory completion of applicable training to meet requirements of regulating bodies and demonstrated ability to apply such training to the work environment. Lived experiences with behavioral health issues are valued.
- Possess a valid Michigan driver’s license and have a satisfactory driving record.
