Physical Therapist Assistant (PTA) | Home Visits | Holland Area

Atrio Home Care
, a partnership between Holland Home, Brio, and Resthaven, is seeking a compassionate and driven Physical Therapist Assistant (PTA) to join our Holland/Lakeshore team. In this role, you'll provide high-quality, one-on-one care to clients in their homes within approximately a 40-mile radius of Holland, MI.

What You'll Do
  • Travel to client homes or care facilities to complete follow-up therapy visits
  • Deliver physical therapy services under the supervision of a licensed Physical Therapist, including implementing restorative care and home exercise programs
  • Complete accurate clinical documentation and progress notes in Matrix Care
    , ensuring compliance with agency policies and HIPAA standards
  • Collaborate with therapists and the interdisciplinary team to support excellent patient outcomes and smooth discharge planning
  • Educate clients, families, and caregivers on therapy techniques and ongoing care needs
  • Assist with therapy equipment management, program development, and participate in team trainings and goal setting initiatives
  • We now have a lot of Ortho cases- selling point
  • They only have routine follow-up visits, no oasis; helps with less medically complex pts
  • 6 visits minimum or 30 visits per week
Schedule & Location
  • Hours: Full or Part-time opportunities available
  • Schedule: Monday–Friday (no weekends, holidays, or on-call requirements)
  • Shift: 8:00 AM – 4:30 PM
  • Coverage Area: Holland /Lakeshore team
Qualifications
  • Active Michigan PTA license required
  • 1+ year of PTA experience preferred
  • Experience working with geriatric or medically complex populations strongly preferred
  • Strong ability to work independently while also collaborating as part of a team
  • Excellent time management and organizational skills
