As a Home Health Physical Therapist, you will provide one-on-one care in patients' homes, helping them improve mobility, regain strength, and achieve greater independence while making a meaningful difference in their lives.

• Evaluate patients and develop individualized treatment plans.
• Provide therapy services to improve strength, mobility, balance, and function.
• Educate patients and caregivers on exercises and safety.
• Document visits and patient progress in the EMR.
• Collaborate with physicians and the interdisciplinary care team.

• Graduate of an accredited Physical Therapy program
• Current Physical Therapist license in the State of Michigan
• Current CPR certification
• Valid driver's license, reliable transportation, and auto insurance
• Minimum of one (1) year of Physical Therapy experience preferred
• Home health experience preferred, but not required
• Ability to work independently and manage time effectively
• Strong communication and interpersonal skills
• Proficient in electronic medical records (EMR) and documentation
