Position SummaryAs a Home Health Physical Therapist, you will provide one-on-one care in patients' homes, helping them improve mobility, regain strength, and achieve greater independence while making a meaningful difference in their lives.
Responsibilities- Evaluate patients and develop individualized treatment plans.
- Provide therapy services to improve strength, mobility, balance, and function.
- Educate patients and caregivers on exercises and safety.
- Document visits and patient progress in the EMR.
- Collaborate with physicians and the interdisciplinary care team.

Qualifications- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license in the State of Michigan
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
- Minimum of one (1) year of Physical Therapy experience preferred
- Home health experience preferred, but not required
- Ability to work independently and manage time effectively
- Strong communication and interpersonal skills
- Proficient in electronic medical records (EMR) and documentation
