Under general direction, the Public Health Planner is responsible for planning and organizing public health programming at the local and regional community level. This position will support further development of a Community Information Exchange (CIE) to enhance connectivity of organizations addressing health-related social care needs. Working in partnership, this position will support improving access to healthcare by reducing barriers, increasing connectivity and supports the implementation of community health needs assessment and implements strategies to promote active involvement of community members and stakeholders in developing and driving community health initiatives to promote individual and population health specific to Rural Health Transformation initiatives.

This position is in the office and community based work with travel across the 10 county jurisdiction required.

• Graduate degree in public health, health promotion, community health, or related field required; or a Bachelor’s degree in health education/promotion, public health or a related field with three years’ experience in health education/community health or public health field.
• In-depth knowledge of strategies and processes to promote public health, and ability to plan and organize development and implementation of community health needs assessments and health improvement plans at a level normally acquired through three years progressively more responsible, related experience.
• Commitment to professional development as shown through attainment of recognized certifications. CHES or MCHES certification preferred.
• Must have emotional and mental maturity necessary for establishing and maintaining good working relationships with clients, community partners and agency personnel. Ability to work effectively with the public and peers on projects requiring a team approach.
• Good written and oral communication skills; excellent public speaking and group facilitation skills.
• Reliable transportation with valid Michigan driver’s license and certificate of insurance to travel to other locations for work, training, or meetings.
