What You’ll Do
  • Travel to patient homes and facilities within the assigned service area
  • Assist patients with activities of daily living (bathing, dressing, grooming, toileting)
  • Support safe transfer, ambulation, positioning, and comfort measures
  • Assist with meals, hydration, and basic care‑related homemaking
  • Take and document vital signs as directed
  • Observe and report changes in patient condition to the RN
  • Follow individualized Hospice Care Plans while adapting to assignment changes and varying care environments
  • Document care provided and communicate professionally with patients, families, facility personnel, and the care team
Qualifications
  • Must hold current certification as a nurse aide and be in good standing on the state registry
  • Minimum 1 year of experience as a hospice aide, home health aide, or certified nursing assistant preferred
  • Valid driver's license and reliable transportation with valid insurance
  • Willingness to travel frequently within the assigned service area
  • Ability to read, write, and communicate effectively in English
  • Ability to perform the physical demands of the role, including standing, bending, lifting, and use of transfer equipment
Schedule

Full Time:
Monday‑Friday, 8:00 AM - 5:00 PM, with weekend rotation

Part Time:
Three weekdays (8:00 AM - 5:00 PM), with weekend rotation

Per Diem:
Flexible weekdays, Monday‑Friday (8:00 AM - 5:00 PM), with weekend rotation

Pay Range

$18—$22 USD

Optimal Care conducts a background screening upon acceptance of a contingent job offer. Background screening is completed by a third‑party administrator, the Michigan Long‑Term Care Partnership, and is performed in compliance with the Fair Credit Report Act.
