Hospice Admission Registered Nurse / RN

Make an impact by enabling patients to spend quality time with loved ones and ensuring they receive compassionate hospice care. You will assess hospice eligibility, explain services to patients, families, and caregivers, and address initial patient needs.

• Enable patients to spend quality time with their loved ones and doing the activities they enjoy.
• Assess patients for hospice eligibility and explain hospice care services to patients, families/caregivers.
• Ensure all patient’s initial needs are addressed.
• Collaborate with Inter‑Disciplinary Team members regarding initial plan of care, patient’s condition, and other essential information.

• RN license in the state you work.
• Two years of experience as a registered nurse; hospice experience preferred.
• Current driver’s license.
• Current CPR and BCLS certification.
