Salary Range: $39.00 - $44.00 Hourly

This full-time position offers flexibility in scheduling. For example, a typical schedule may start around 2:30 PM on Friday and continue through Monday morning (around 10:00 AM), but we are open to adjusting hours to fit the right candidate while maintaining weekend coverage.

• This role includes on-call responsibilities, along with completing scheduled visits as needed.

• Conduct comprehensive patient assessments and develop individualized plans of care
• Provide skilled nursing care in accordance with hospice standards and physician orders
• Coordinate care with physicians, caregivers, and interdisciplinary team members
• Educate patients and families on symptom management, disease progression, and end-of-life care
• Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase)
• Participate in interdisciplinary team meetings and care planning
• Support after-hours patient needs as part of an on-call rotation when applicable

• Active Registered Nurse (RN) license
• Hospice or home health experience preferred
• Strong clinical assessment and care coordination skills
• Ability to work independently in the field
• Strong communication and documentation skills
• Experience using HCHB (Homecare Homebase) preferred

• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
