Summary:

Part‑time position, 24 hours a week. Two days as Nurse Supervisor, one day as Prenatal Nurse.

Overview

The Nurse Supervisor is part of a clinical team and works at the top of their licensure to enhance the safety, effectiveness, and efficiency of care through both direct nursing responsibilities and delegation of technical and clerical functions. The primary care Prenatal Nurse is a registered nurse who specializes in providing high‑quality comprehensive care to individuals throughout the lifespan, including childbearing individuals, pregnant and postpartum persons, and their families.

and Responsibilities

Nurse Supervisor (Part of FTE)

  • Comply with all CHC policies and procedures as described in the medical department policy and procedure manual.
  • Ensure compliance with regulatory standards including JCAHO, OSHA/EOC, Safety/Infection Control, Quality Care Audits, DPH licensure requirements.
  • Supervise medical assistants and staff nurses, ensuring compliance with agency policy regarding telephonic patient instructions and triage.
  • Assist in enrolling Complex Care Management patients, enhancing patient experience and clinical outcomes.
  • Consult with Regional Nurse Manager and CNO on nursing problems and agency policy interpretation.
  • Plan and organize orientation and in‑service training for nursing and medical assistant staff.
  • Allocate appointments according to CHC protocols, collaborating with Operation Managers to adjust schedules.
  • Interview, hire, and appraise performance of medical assistants and nurses, addressing concerns and rewarding behaviors.
  • Coach and supervise direct reports, facilitate team approach, and manage challenging or emotional customer situations.
  • Participate in community health awareness programs and events as able.
  • Manage site budget, coordinate care with community based providers, and ensure safe, accurate collection of biologic specimens.
  • Supervise medical assistant externs and nursing students, ensuring appropriate clinical education.
  • Support primary care under the planned care model, collaborating with PCP for preventive care and care coordination.
  • Assist providers with exams, treatments, and procedures; administer medications and monitor patient responses.
  • Document all aspects of patient care in the medical chart accurately and timely.
  • Maintain strict infection control standards, support medical assistants with patient preparation, and follow up abnormal screening results.
  • Maintain compliance with mandatory annual training and expert use of electronic health record.

Primary Care Prenatal Nurse (Part of FTE)

  • Support primary care under the planned care model, including pre‑visit planning, daily huddles, and nurse‑led visits.
  • Triage patients based on clear protocols, both in person and over the phone.
  • Collaborate with PCP to identify needs for preventive care and chronic disease management.
  • Provide health education during visits and telephone calls, aiding patients in self‑management of goals.
  • Coordinate care with community providers and attend Continuation of Care Collaboration meetings.
  • Follow up on emergency room visits and hospital discharges, ensuring OB discharge follow‑up.
  • Assist with exams, treatments, and procedures; administer medications, monitor responses, and reconcile medications.
  • Perform nursing visits under standing orders or delegated visits (prenatal intakes, uncomplicated UTIs, insulin titration, vulvovaginal candidiasis, pregnancy testing, etc.).
  • Document all patient care accurately in the medical chart on a timely basis.
  • Maintain strict infection control and workstation standards, monitor abnormal screening results, and order supplies as necessary.
  • Participate in Project ECHO complex care management and Integrated Care Team meetings.
Qualifications
  • Registered Nurse, BSN (required). MS in nursing or related field (preferred).
  • Driver’s license.
  • Electronic health record experience.
  • Two or more years of ambulatory nursing experience (preferred).
  • Prior management experience in healthcare.
  • Bilingual preferred.
  • Computer skills:
    Microsoft Word, basic Excel, PowerPoint.
  • CCCTM certification (certified clinical case management).
