Pay: $42.50 - $48.00/HR
Schedule:
Monday 8:00 AM‑4:30 PM on‑call rotation
The Eligibility Specialist RN, under the direction and supervision of the Clinic Supervisor, provides skilled nursing care to participants, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops an appropriate plan of care, teaches participants, caregivers and families, and communicates patient status to primary caregivers, physicians and the Interdisciplinary Team.
Required Qualifications- Graduate of a school of professional nursing
- Current state Registered Nurse (RN) license
- Cardiopulmonary Resuscitation (CPR) certification required
- Minimum one year of experience working with frail or elderly populations
- Valid driver’s license, proof of insurance and means of transportation
- Medical clearance for communicable diseases and up‑to‑date immunizations before direct participant contact
- Bachelor of Science in Nursing (BSN) preferred
- Conducts in‑person comprehensive initial nursing assessments with new PACE enrollees and coordinates with the Interdisciplinary Team to develop comprehensive care plans
- Conducts in‑person nursing re‑assessments semi‑annually and as needed
- Active member of the Interdisciplinary Team, attends meetings, communicates participant changes, collaborates on care plans and coordinates 24‑hour care delivery
- Provides orientation, initial and annual competencies for LPNs, CNAs and Clinic Clerks as needed
- Performs competencies on appropriate day‑center staff as required
- Provides general nursing care to participants, performs or supervises medication administration and treatments in accordance with nursing standards
- Triages participants to determine priority of care
PACE is a Program of All‑Inclusive Care for the Elderly. The team includes clinical professionals, housekeeping, maintenance, culinary staff and other support roles. Positions offer flexible schedules, occasional weekend work, and opportunities for meaningful relationships with patients.
