Pay: $27.55 - $32.50

Monday-Friday 8:00 AM-4:30 PM

The LPN Med Room Under the direction and supervision of the Clinical Care Manager and in coordination with the Medication Technician, is responsible for the effective management and provision of participant medications provided via the PACE medication department. Administers medications, implements primary care orders, and evaluates participant needs physically and/or by telephone. Provides education to Senior Community Care participants and caregivers regarding the plan of care and medications.

Communicates with participants’ primary care givers, physicians, clinic staff and interdisciplinary team.

• Graduate of a school of professional nursing required
• Current Practical Nurses License (L.P.N.)
• Cardio Pulmonary Resuscitation (CPR) certification required
• Minimum one year’s experience working with the frail or elderly population
• Valid driver’s license, proof of insurance and means of transportation
• Medical clearance for communicable diseases and up-to-date immunizations before direct participant contact

• Intravenous Venous (IV) Certification preferred
• Processing of medication orders for all participants, ensuring complete, accurate, and legal prescriptions are received by the dispensing pharmacy
• Review notes from outside providers and process medication orders with approval of PACE provider
• Coordinate maintenance of participant medication schedules, including implementation of new orders, mid-cycle changes, discontinuations, and reconciliations through transition processes
• Medication reconciliation through care setting transitions (LTC/Hospital/ALF discharge) in accordance with organization procedures and facilitate required changes
• Communicate with and between PACE providers, community-based providers, and the pharmacy regarding medication orders, clarifications, and pharmacy recommendations
