About Optimal Care & Role Overview

Optimal Care is a leading provider of Physician Services, Home Health, and Hospice that empowers its staff to deliver compassionate, personalized care. We are building a talent pipeline for future Hospice RN Case Manager roles in the Metro Detroit area. As a Hospice RN Case Manager, you will provide skilled nursing care and compassionate support to patients and families during life's final chapter, ensuring comfort, dignity, and quality of life.

Responsibilities

• Build relationships with patients, families, physicians, facilities, and interdisciplinary team members.

• Coordinate comprehensive care for your caseload, addressing physical, emotional, and spiritual needs.

• Provide skilled nursing care in patients’ homes, assisted living facilities, and nursing homes, requiring daily travel within the service area.

  • Office:
    Bingham Farms, MI
  • Main Service Areas:
    Genesee, Livingston, Macomb/Oakland, Wayne
Working Hours
  • Full‑Time:
    Monday–Friday, 8 a.m.–5 p.m., weekend on‑call rotation
  • Part‑Time:
    Three weekdays, 8 a.m.–5 p.m., weekend on‑call rotation
  • Per Diem:
    Flexible weekdays, Monday–Friday, 8 a.m.–5 p.m., weekend on‑call rotation
Required Qualifications
  • Graduate of an accredited diploma, associate, or bachelor’s school of nursing
  • Current state license as a Registered Nurse
  • Minimum one (1) year of clinical nursing experience
  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage
Preferred Qualifications
  • Bachelor of Science in Nursing (BSN)
  • Hospice or home health experience
Essential Skills and Competencies
  • Strong nursing skills grounded in accepted standards
  • Ability to assess and respond to patient, family, and caregiver needs in diverse settings
  • Skill in coping with family emotional stress and tolerance of varied lifestyles
  • Ability to juggle multiple priorities, documentation, and deadlines
Pay Range

$77,000 – $83,000 USD
