Monday 8:00 AM-4:30 PM on-call rotation
The Eligibility Specialist RN, under the direction and supervision of the Clinic Supervisor, provides skilled nursing care to participants, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops an appropriate plan of care, provides participant, caregiver and family teaching, and communicates participant’s condition to primary caregivers, physicians and the Interdisciplinary Team.
Responsibilities• Conduct in‑person comprehensive initial nursing assessments with new PACE enrollees and coordinate with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
• Conduct in‑person nursing re‑assessments semi‑annually and as needed.
• Function as a member of the Interdisciplinary Team, maintaining regular attendance at meetings and communicating participant changes, collaborating on plan‑of‑care decisions, and coordinating 24‑hour care delivery.
• Provide orientation, initial and annual competencies to Licensed Practical Nurses (LPN), Certified Nursing Assistants (CNA), and Clinic Clerks, and perform competencies for appropriate day center staff as needed.
• Perform general nursing care to participants, including administration of prescribed medications and treatments in accordance with nursing standards.
• Triage participants to determine priority of care.
- Graduate of a school of professional nursing.
- Current state Registered Nurse (R.N.) license.
- Cardiopulmonary Resuscitation (CPR) certification required.
- Minimum one year’s experience working with frail or elderly population.
- Valid driver’s license, proof of insurance and means of transportation.
- Medical clearance for communicable diseases and up‑to‑date immunizations before direct participant contact.
- Bachelor of Science in Nursing (BSN) preferred.
- Experience conducting comprehensive initial assessments and semi‑annual re‑assessments for PACE enrollees.
- Experience collaborating within an Interdisciplinary Team for 24‑hour care delivery.
- Experience providing orientation and competency training to nursing and support staff.
