RN - Home Health

We are seeking a dedicated and experienced Registered Nurse to join our Home Health team. The nurse will be responsible for providing comprehensive care to adult and geriatric patients in their homes, focusing on patient assessment, education, and management of procedures such as IV therapy, tracheostomy care, and wound management.

• Conduct admission assessments and develop patient care plans.
• Manage peripheral IVs, central lines, and feeding tubes.
• Perform clinical assessments and maintain accurate documentation.
• Provide care for colostomies, ileostomies, and tracheostomies.
• Assist with patient education and discharge planning.
• Maintain compliance with OASIS documentation and Medicare guidelines.

• Minimum of 1 year of prior charge experience in home health.
• Active RN license in Michigan (compact licenses not accepted).
• Proficient in Epic charting system.
• Current certifications in BLS required; AHA preferred.
• Experience with wound care and IV maintenance is necessary.
