About the Role
Signify Health, part of CVS Health, is looking for a part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The role involves engaging with people in the comfort of their own homes, conducting physical evaluations, reviewing medication and medical history, and performing diagnostic tests such as diabetic eye exams and spirometry when ordered.

- Work with Signify Health as a clinician colleague, fulfilling the specific requirements of this role.
- Be flexible to travel locally and within licensed states as needed.
- Conduct virtual visits when required.
- Provide all Signify Health services indicated, including Diagnostic & Preventive Services products such as spirometry and DEE.
- Active, unrestricted license(s) in covered area(s).
- Board certification (required).
- Multi‑state licenses (preferred).
- Ability to cover primary locations:
Sault Ste. Marie (Counties: Chippawa, Luce, Mackinac).
- Multi‑state licenses.
- Experience conducting in‑home evaluations.
Primary location:
Sault Ste. Marie (Counties: Chippawa, Luce, Mackinac). Anticipated weekly hours: 29.5. Time type: part‑time. Travel may be required locally and within licensed states.
- Pay range: $41.85 – $90.13 (based on experience, education, geography).
- Health plan members may be in homes with pets, secondhand smoke, and other allergens; clinicians must work safely in these environments.
