Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations in the Marquette, Michigan market. The role enables clinicians to deliver comprehensive, personalized care directly to members in the comfort of their homes, using an iPad and an intuitive clinical workflow.

• Conduct 45‑minute in‑home visits: gather medication and medical history, perform a physical assessment, and order additional diagnostic tests as needed.
• Use an iPad and clinical workflow to complete evaluations without prescribing medication, ordering lab tests, or altering existing treatment plans.
• Provide diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
• Travel locally and within licensed states as required, and conduct virtual visits when necessary.
• Collaborate with Signify Health colleagues and meet role‑specific requirements.
• Maintain accurate documentation and comply with all clinical and regulatory standards.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).

Hourly base rate or base annual full‑time salary: $41.85 – $90.13. Compensation will vary based on experience, education, geography, and other factors.

Anticipated weekly hours: 12; part‑time. Flexible schedule with per‑visit incentives.
