Job Title

This role offers an opportunity to collaborate with staff members to support seamless patient care. You will make a meaningful impact serving as a trusted partner to patients and families navigating health care needs.
Coordinates and delivers primary care in collaboration with the Practice Site Physician by:
- Working within their approved scope of practice.
- Obtaining complete medical history and physical data on patients.
- Interpreting and integrating data to determine appropriate diagnostic and therapeutic procedures as needed.
- Performing a complete physical exam and recording findings. Collaborating with physicians in managing the medical needs of patients.
- Order appropriate laboratory and diagnostic procedures.
- Educating and advising patients regarding health and illness prevention. Recommending community resources to meet patient and family needs.
- Support patient access and care coordination through triage, follow-up, and management of urgent situations
Provides clinical and administrative assistance to BMG programs, e.g., education, follow-up, and the like by:
- Participating in health education.
- Maintaining related records and statistical data and preparing statistical reports as needed.
- Participating in interdisciplinary care conferences and assisting with referrals and dissemination of information to appropriate vendors.
- Assisting in the development and coordination of ongoing education and follow-up programs designed to measure client outcomes.
Required Credential(s):
- Graduate of an accredited Physician Assistant or Nurse Practitioner program.
- Current, valid state licensure and appropriate certification (NCCPA or ANCC/AANP) obtained prior to hire date or job transfer date. Verified by local Credentialing Office.
- BLS obtained within 1 Month (30 days) of hire date or job transfer date if required. American Heart Association or American Red Cross accepted.
- Current BLS and ACLS certifications if required.
- Ability to work collaboratively in a fast-paced, team-oriented environment.
