Home Health Registered Nurse
Position available for a Home Health Registered Nurse. Shift is from 08:00 to 16:30, with every 5th weekend required. No float or call required. Candidates must be proficient in PICC, Port, Trach, Pleurx, and patient education. Familiarity with Medicare guidelines related to Homecare is required. Ability to complete OASIS documentation is necessary. Required certifications include BLS and Adult Home Health experience.
Epic experience is mandatory. Previous charge experience and a minimum of 1 year of experience are required. Unit will not accept first-time travelers. Patient age groups include adults and geriatrics. Daily census is 16-20 patients per nurse. The average caseload is 5-6 visits per day. Interdisciplinary support includes Physical Therapy, Interpretation Services, and Social Services. Candidates must demonstrate skills in admission assessment, clinical documentation, wound care, and tracheostomy care among others.
