RN - Home Health

Home Health Registered Nurse (RN)

This is a 13-week contract position.

Day shift, 5x8 hours (08:00 - 16:30), with every 5th weekend required.

We are seeking a dedicated and skilled Home Health Registered Nurse (RN) to provide exceptional care to patients in their homes. As an RN in home health, you will play a crucial role in managing patients' health conditions, providing education, and ensuring adherence to care plans.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Manage and administer medications and treatments, ensuring compliance with medical guidelines.
• Educate patients and families on health management and care practices.
• Document patient care activities and complete required OASIS documentation.
• Provide care for patients with complex needs, including those requiring PICC lines, ports, trach, and Pleurx management.
• Collaborate with interdisciplinary teams, including Physical Therapy and Social Services.

Required Experience / Certifications / Licensure

• Current active Michigan RN license.
• At least 1 year of home health experience required.
• Proficiency in using Epic for patient documentation.
• Basic Life Support (BLS) certification required.
• Previous charge experience is preferred.
