Residential programs span apartments, single-family homes, and services provided in the individual’s own home. Turning Leaf’s residential, community integration, skill-building, day treatment, and therapeutic services emphasize practical life skills that support independence and self-determination at every phase of recovery.

This is a full-time, on-site Case Manager role based in Lansing, MI. The Case Manager will coordinate individualized service plans, complete assessments, and monitor progress to support each person’s recovery and community integration. Daily responsibilities include linking individuals to appropriate resources, facilitating access to residential and day treatment programs, and collaborating closely with clinical staff, families, and external agencies. The role also involves maintaining accurate documentation, ensuring compliance with organizational and regulatory standards, and advocating for the needs and preferences of individuals served.

The Case Manager will use a person-centered, strengths-based approach to promote life skills, independence, and stability.

• Candidates should possess strong case management and care coordination skills, including assessment, service planning, and progress monitoring.
• Candidates should possess skills in behavioral health support, such as knowledge of recovery principles, community integration, and life skills coaching.
• Candidates should possess interpersonal and communication skills, including active listening, collaboration with multidisciplinary teams, and respectful engagement with individuals and families.
• Candidates should possess organizational and documentation skills, including accurate record-keeping, time management, and attention to regulatory and agency requirements.
• Relevant qualifications include a bachelor’s degree in social work, psychology, human services, or a related field (or equivalent experience); experience in behavioral health or residential services is preferred.
• Additional beneficial qualifications include familiarity with community resources in Lansing, MI, crisis intervention skills, valid driver’s license with reliable transportation, and the ability to work flexible hours as needed.
