General Description

The Supports Coordinator / Case Manager has primary responsibility for assisting eligible consumers with the development of a Person/Family Centered Plan (P/FCP) of Service. The Supports Coordinator / Case Manager works with the consumer and his/her chosen support network in developing the P/FCP to design a plan that promotes individual choice, the use of community resources, and formal and informal supports in pursuit of the individual's desired future.

The Supports Coordinator / Case Manager assists the consumer in the assignment of available agency resources to the plan. The Supports Coordinator / Case Manager ensures the monitoring of supports and services according to the plan.

In addition, the Supports Coordinator / Case Manager manages agency resources allocated through the Utilization Management System and the P/FCP. The Supports Coordinator / Case Manager provides the agency with data required for determining consumer satisfaction, treatment and service outcomes and effective, efficient utilization of agency resources. The Supports Coordinator / Case Manager is assigned to work with a specific specialty population, i.e. adults with Severe Mental Illness (SMI), elderly adults with SMI, children and youth with Severe Emotional Disturbances (SED) or adult or children consumers with Intellectual/Developmental Disabilities (I/DD), and individuals who have Substance Use Disorders.

Minimum Qualifications

• Bachelor's degree from an accredited program in Social Work.

• Licensed by the State of Michigan to practice as a Social Worker.

Competencies Adults with SMI

• Education, training and/or work experience in the behavioral health field with adults diagnosed with severe and persistent mental illness.

General Position Competencies

• Ability to communicate verbally and in writing effectively.

• Ability to meet at times and locations chosen by the consumer.

• Possess or acquire ability to use computers in a clinical setting (or a non-clinical setting such as in the community or at a consumer's home).

• Familiarity with managed care, state, federal, and private health insurance benefits.

• Basic reading and arithmetic skills are required.

• Excellent customer service skills are required.

Attention to detail and excellent organizational skills are required.

• The ability to demonstrate absolute discretion with confidential information is required.

• Experience with Microsoft Office applications (Word, Excel, Power Point, etc.) is highly desirable.

• Experience using office equipment (including, but not limited to, desktop telephone, computer, copier, fax machine, scanner, laser printer, and desktop calculator).

Essential Duties and Responsibilities

1. Assist a consumer with developing his/her Person/Family Centered Plan to include proactive plans for future needs, and incorporate community and other external resources into the plan to meet the consumer's long-term needs.

2. Assist the consumer in scheduling Person/Family Centered Planning meetings at a time and location convenient for the consumer and his/her chosen planning team, and act as the meeting facilitator at the consumer's request.

3. Assist a consumer to monitor and update his/her Person/Family Centered Plan (P/FCP) and perform periodic consumer satisfaction assessments as specified (on at least an annual basis).
