Responsibilities What You'll DoAs our Start‑of‑Care RN, you'll be the one who kicks off each patient's journey with us. Your role is vital, meaningful, and full of impact. You will:
- Complete start‑of‑care assessments with accuracy, compassion, and confidence
- Build trust with patients and families from the very first visit
- Develop individualized care plans that set patients up for success
- Coordinate with physicians, therapists, and the clinical team to ensure seamless transitions
- Document efficiently using our supportive EMR system
- Serve as a clinical resource and role model for the team
- Bring positivity, professionalism, and a can‑do attitude to every day

Requirements What You Bring- Active RN license
- Strong clinical assessment skills
- Home health or start‑of‑care experience preferred, but we'll train the right superstar
- Excellent communication and relationship‑building abilities
