Nurse Practitioner or Physician Assistant

Provide direct primary care to patients, with emphasis on geriatric care, including treatments and medications. Locations of care include clinic, nursing home, hospice, assisted living and patients’ homes.

• Conduct comprehensive health assessments and physical examinations for patients.
• Complete recertifications as required by Medicare.
• Develop and implement individualized, long‑term care plans.
• Manage chronic diseases and address acute medical issues.
• Prescribe medical care to patients, including treatments and medications.
• Collaborate with multidisciplinary teams to coordinate patient care.
• Monitor patient progress and adjust care plans as needed.

Education: Master’s degree in Nursing

Experience: 1‑3 years nurse practitioner experience preferred

• Valid State of Wyoming authorization as a Nurse Practitioner.
• Prescriptive authority in the State of Wyoming.
• National certification in degree specialty.
• For certain specialized positions, a DEA registry license may be required.
• Valid CPR certification (cardiopulmonary resuscitation).
• Valid BLS certification (basic life support).

Education: Master’s degree

Experience: 3 years as a Physician’s Assistant

• National and valid State of Wyoming certification as a Physician’s Assistant.
• Prescriptive authority in the State of Wyoming.
• Licensure by the Wyoming Board of Medicine.

* The level of this position will be determined based on the selected candidate’s experience.

• 3 Years of Geriatric Experience
• Certification or Focused Training in Geriatric Care
• Experience in Providing Geriatric Care
• Experience Working in Assisted Living Facilities and/or Skilled Nursing Facilities
• FQHC Experience
• Healthcare Student Teaching Experience
• PCMH Experience
