Do you have a passion for helping school-age students reach new heights? Do you find yourself cheerleading others as they work towards reaching their goals? Do you love helping students explore what options await them as they grow up? The Wyoming Division of Vocational Rehabilitation is looking for a creative and highly motivated individual to support students as they navigate the transition process of career exploration.
The Wyoming DIF Transition Navigator is responsible for the recruitment and case management of students enrolled in the Pathways to Progress program. The Transition Navigator is the primary point of contact for the coordination of services within the P2P program, which is provided by the Centers for Independent Living (CILs), mental health providers, and DVR Pre‑employment Transition Services (Pre‑ETS). The P2P Transition Navigator offers creative avenues for students to engage in career exploration through both individual and group activities.
This position is responsible for the development and facilitation of local transition council teams to support the P2P program and efforts.

ESSENTIAL FUNCTIONS- Recruiting eligible students for the P2P program
- Participating in outreach activities to promote the P2P program
- Working collaboratively with stakeholders, assigned school districts, and Centers for Independent Living
- Providing creative and innovative opportunities for students to engage in career exploration
- Networking with local employers to create positive relationships and opportunities for student work experience sites
- Providing basic case management for students enrolled in the P2P program to ensure they have access to partner services and mental health care as applicable
- Serving as a mentor to students by offering support, encouragement, and inspiration as they explore their futures
- Preference for a Bachelor’s degree in a human service‑related field.
- Preference may be considered for an Associate’s degree in the human service field, and extensive experience.
- Preference for one year of experience working with youth in secondary school or providing Pre‑Employment Transition Services to students with disabilities. Preference may be considered for equivalent combinations of related education and experience.
- Knowledge of IEPs, 504 Plans, and regulations
- Ability to work in cooperation with Local Education Agencies, DVR, and Workforce offices
- Ability to be proactive, organized, and manage caseloads
- Knowledge of informed choice, confidentiality, and conflict of interest;
Knowledge of disability rights, culture, and history - Skill in working with individuals with disabilities, as well as skill in sensitive, respectful, and effective communication with people of diverse backgrounds, cultures, and languages
- Skill in public speaking
- Ability to work within your scope of practice, knowledge, skill, and abilities and an ability to delegate, refer, or communicate when a duty or role is outside your scope of practice or skill level
