Our client, a healthcare institution committed to excellence, is seeking a Clinical Pharmacist to join the team. The Clinical Pharmacist will play a vital role in patient care, applying advanced pharmaceutical knowledge and clinical expertise to ensure optimal medication management.
Requirements:- Pharm.
D. or residency from an accredited institution. - PGY 1 Residency in Hospital.
- Licensed to practice in the State of Texas.
- A.C.P.E. accredited I.V. Certification, Completion of an accredited Residency in a Hospital Pharmacy (PGY1/PGY2).
- Basic Life Support (BLS) certification.
- Encouraged:
Any Board of Pharmacy Specialty certifications. - Preferred: BCPS - Board Certified Pharmacotherapy Specialist or any board certifications.
- Familiarity with standard concepts, practices, and procedures within a particular field.
- Medication Management:
Provide clinical pharmacy services to optimize medication therapy, ensuring safe and effective drug utilization. - Patient Care:
Collaborate with healthcare providers to develop and implement patient-specific medication treatment plans, considering patient needs and therapeutic goals. - I.V. Certification:
Maintain A.C.P.E. accredited I.V. Certification and ensure compliance with standards for intravenous medication preparation. - Residency Training:
Apply knowledge and skills acquired through completion of an accredited Residency in a Hospital Pharmacy (PGY1/PGY2) to enhance patient care. - Board Certification:
Pursue and maintain relevant Board of Pharmacy Specialty certifications, with preference for BCPS or other board certifications. - Compliance:
Ensure compliance with state regulations and licensing requirements, practicing within the scope of licensure. - Education:
Provide education to healthcare professionals, patients, and caregivers on medication therapy and management. - Quality Improvement:
Participate in continuous quality improvement initiatives, evaluating and enhancing pharmacy services.
